Well, for the insulin it's just normal since it can be hard to calculate the amount you should inject yourself with. Every meal you eat has its own calculation and injection, if you inject yourself with too much insulin you will have a mild to moderate hyperglycemia (sugar in your blood gets too low). Speaking as a type 1 diabetic, (juvenile, insulin dependent) a person whom controls their sugar level well should have about 4 hypoglycemia a week. But the problem is when you inject yourself with the wrong type of insulin (slow acting, medium and fast acting) since insulinotherapy usually uses more than one type, or also if you inject yourself twice the same dose. But these problems usually occur with people who are older or new diabetics... Or in a case that was on digg a while ago where a wal-mart pharmacy gave the wrong type of insulin to someone which resulted in the death of the patient. This is probably why insulin is now on the list.

Except when we needed that type of questioning, like the lead-up of the Iraq war. Instead of questioning it was more like a White House foil, just like every show back then (except a few).
